Abstract　： 「かみ合わせ外来」は, 平成13年4月に開設された神奈川歯科大学附属病院特殊外来の一つで, その診療コンセプトは, 咬合に関する問題を有する患者を対象に, 包括的歯科医療を基本姿勢とし, 客観的評価にもとづいた診断および治療計画を立案, 実施し, 患者のQOLを改善, 増進することにある1, 2). 本外来で担当する症例の大部分は, 広範囲におよぶ咬合治療の必要性の他, 咬合治療の既往や医原性が疑われる問題を有する症例も多く, 通常の歯科治療のみならず患者の心理面での評価が必要と思われるものには, 心理テストを含めた専用のプロトコールを作成し実施してきた. しかし, このような慎重な対応による咬合治療にもかかわらず, 治療が進む段階で説明のつかない自覚症状や患者への対応の困難さなどの問題が生じ, 予定通りの診療が進められない状況に直面するケースも多く, これまでの歯科的な患者対応に限界を感じていた. そこで, 平成14年4月より通常の歯科治療以外の, 患者に関する背景要因を把握する目的で, 心身医学の専門歯科医による心身医学, 精神医学的な医学面接(医療面接の中で, より専門性の高い面接)を導入し, また平成16年4月からは精神科医と連携し, リエゾン診療の体制を開始した.

Abstract　： The Division of Occlusion & Craniomandibular Disorder was established in Kanagawa Dental College Hospital at April 2001 ; its clinical concept is to treat patients with problems regarding occlusion, based on comprehensive dental treatment with diagnosis and treatment planning involving objective assessment for occlusion and to improve and maintain patient “QOL”. Despite careful occlusal treatment, however, such patients complained of unexplained subjective symptoms and expressed difficulty regarding dental procedures. More understanding as to background factors of the patient was necessary to achieve dental treatment for these patients. A dental psychosomatist, who is dentist, has been attending medical interviews since April 2002 ; moreover, liaison consultation by a medical psychiatrist was introduced in April 2004 in our hospital. The procedure of comprehensive dental consultation (treatment) is as follows : First, allowance for acute symptoms by pain control ; then, explanation on the consultation (treatment) system, and obtaining of informed consent to our treatment procedures. The treatment consists of four stages : Stage I is basic examination ; stage 2 is specialized examination ; stage 3 is actual occlusal treatment, and stage 4 is postoperative management. Medical interview is essential consultation that should be introduced into dental education, and our system is comprehensive dental consultation (treatment) on behalf of the patient.
