Abstract : 顎関節症は, 運動障害や疼痛によって咀嚼, 発音等の顎機能が損なわれる疾患である1). その病因因子2)については, これまで様々な分野から多数の報告がなされてきた. 歯科領域では病因因子として咬合の関与が強調されてきたが, 顎関節症は多因子性の疾患であり, 咬合は複数の病因の中の一つの因子として関与している可能性が指摘されはじめている. しかしながら, どのような咬合が顎関節に傷害的に働くのかという点については不明な点が多い. 本研究では, 両側性の非復位性顎関節円板転位を持つ症例群の下顎頭運動, 咬合接触部位を調べ, どのような特徴があるかを明らかにしようとした. 方法 1. 分析対象者 神奈川歯科大学附属病院矯正科に来院した不正咬合者24名を対象とし, その症例分析資料, MRI画像, アキシオグラフ(Gamma社, Austria)による顎運動記録, SAM咬合器(Gamma社, Austria)付着模型を診査に用いた. これらはすべて矯正治療のために採得されたものである.

Title : Occlusal Contact During Bruxism-Like Grinding Jaw Movement in a Case of Disk Displacement without Reduction

Abstract : TMD (temporomandibular disorder) is a disease of disordered mastication and pronunciation by motor disturbance and various pain. There have been many experimental studies and various reports as to the etiological factors. It has been put emphasis on a occlusion in dental domain, but they suggested TMD has many etiological factors, and a occlusion is one of them. However, there have been few reports as to what kind of occlusion injures TMJ. The purpose of this study is to declare what occlusion has reference to a alteration of stroma in TMJ, through the investigation of patients with occlusion, who have both sides anterior disk displacement without reduction. (ADDw/oR) 1) Quantity of condylar lateral motion at bruxism The bilateral no-symptom group was 0.204±0.155 (mm), the group of bilateral ADDw/oR was 0.385±0.286 (mm). The latter was evidently large. (p< 0.01) 2) About the route of condylar movement at bruxism We weighed lateral movement with bruxism movement by the classification of Tamaki et al. In no-symptom group there were tendencies to be more comformity patterns on the horizontal plane. In the no-symptom group, there were more comformity patterns than lateral patterns, but in the ADDw/ oR group, there were more lateral patterns than comformity patterns on the sagittal plane. 3) About maxillary guide points and interferance We recorded the guide of lateral movement. In the no-symptom group, there were many maxillary guide points at the lateral incisor, canine, and first pre-molar. In the group of bilateral ADDw/oR, there were many maxillary guide points at the first pre-molar, second pre-molar, and the first molar ; that is to say, guide points in this group were more back than no-symptom group. In the ADDw/oR group, there was much interference on balancing sides than no-symptom group.
