Abstract : 「緒言」 顎口腔領域に生じる病変の中で顎嚢胞の発生頻度は極めて高く, 中でも歯原性嚢胞がその大半を占めている. 歯原性嚢胞の発生原因は, 歯の形成後に消失すべき歯原性上皮の遺残が増殖するためと考えられている. 従来, このような上皮消失のメカニズムは不明とされていたが, 近年個体の発生や成熟個体における生体の恒常性維持などにapoptosisが深く関与していることが明らかにされてきた. 秋山は歯原性上皮の消失メカニズムがapoptosisと考え, この異常に基づく歯原性上皮の残存が歯原性嚢胞の原因となる可能性を想定して, 実験を行った. その結果, 胎児のエナメル器を完成した歯堤上皮や不規則に増殖したエナメル上皮などにはapoptosisの所見が認められたが, Malassezの上皮遺残や歯根肉芽腫内の上皮には認められなかったことから, 何らかの原因によりapoptosisが抑制された結果, 上皮の増殖が起こり顎嚢胞が形成されると考察した.

Title : An Experimental Study of Cyst Therapy by Inducing Apoptosis to Lining Epithelium

Abstract : [Abstract] The effect of apoptosis on the experimental cyst formation was investigated. 2×3mm of materials were excised from the incisor gingiva of Wister rats, and incubated with vinblastine and cycloheximide for the induction of apoptosis. Small vesicles were observed under the basement membrane at 3 hrs of incubation, and exfoliations of epithelial layers were observed at 48 hrs of incubation. DNA fragmentation as the results of apoptosis were revealed on these degenerating epithelium by TUNEL method. These materials failed to perform the epithelial cysts when they were implanted to their own mandible, although the fresh pieces of gingiva were thoroughly successful to the cyst formation. The fresh gingival materials were implanted under the dorsal fascia of the own rats, and epithelial cysts were formed after 10 days. After 0.1 mg of vinblastine was injected into the cyst cavity, the expression of apoptosis was observed on the epithelium of the cyst wall, and then the cyst was replaced to only fibrous granulation tissue after 3 weeks. These results suggested the possibility of cyst therapy by inducing apoptosis.
