Abstract : | 「緒言」 今まで免疫学的役割が考えられていなかった表皮や粘膜上皮が種々のサイトカインを分泌し, T細胞の成熟や活性化などに関与する等, 上皮性サイトカインの機能的意義が現在明らかにされつつある. 上皮細胞が産生するサイトカインとして最初に報告されたのは, 1981年Lugerらによるkeratinocyte(KC)が分泌するepidermal cell derived thymocyte-activating factor(ETAF)であるが, その後のKupperらの研究によって, 培養KCがIL-1αおよびIL-1βと同一のmRNAと蛋白を発現していること, ヒト由来KCが特に外的刺激を加えなくてもin vitroでIL-1α, IL-1βを分泌することが報告された. 一方, 歯周疾患領域においてもIL-1αおよびIL-1βが歯肉溝滲出液中に存在すること, 種々の歯周病原細菌が高いIL-1誘導能を有することが証明されている. |

Abstract : | Abstract : In this study, we clarified the expression of IL-1α and IL-1β in the human gingival epithelium of the periodontal diseases. At the same time, using the first cultured cells of human gingival tissues, we examined whether individual differences in the mode of appearance of IL-1α and IL-1β in the gingival epitheliums might affect the difference in onset, advance and clinical behavior of the periodontal diseases. Immunohistochemical examination of the gingival tissues removed from seven patients with periodontal diseases revealed the appearance of IL-1α and IL-1β in the gingival epitheliums and some of the inflammatory cells in three patients with adult-type marginal periodontitis. In the other three patients with adult-type marginal periodontitis, IL-1α and IL-1β appeared only in the gingival epitheliums. In the remaining one case of juvenile marginal periodontitis, the appearance of IL-1α and IL-1β was not observed in either gingival epithelium or inflammatory cells. In the first cultured cells of gingival tissues with inflammatory changes obtained from six patients (aged 25-51 years), IL-1α appeared in 1%-73% of the cultured epithelial cells from all the patients. IL-1β appeared in 4%-54% of the cultured epithelial cells from five out of the six patients. By means of the ACAS570 Laser Cytometer, analysis was made of cultured epithelial cells removed from the gingivae of 10 patients between 6 and 21 years old with non-flammatory periodontal tissues. The appearance of IL-1α was observed in all the cultured epitherial cells with the occurrence ratio of 1.34% to 6.99%. After the addition of LPS, the number of cells in which IL-1α appeared increased by 1.34 to 3.34 times the rate for the cultured cells without LPS. The number of the cultured gingival epithelial cells from one patient (a 6-year-old male), which manifested the occurrence of IL-1α increased by 23.31% after the addition of LPS, which was a more marked increase than seen in the cells from the other patients. IL-1β also appeared in the cultured epithelial cells from all of the 10 patients examined. The occurrence ratio between 1.47% and 14.9%. After addition of LPS, the number of cells exhibiting the existence of IL-1β increased 1.43-2.8 times in five out of 10 cases. On the other hand, hardly any increase in the occurrence of IL-1β was noted in the cells from the other five patients including the 6-year-old male, who demonstrated a marked increase in the occurrence of IL-1α. In conclusion, who demonstrated a marked increase in the number of gingival epithelial cells showing the existence of IL-1α and IL-1β differs from individual to individual, and the increase in the number of the IL-1α or IL-1β occurring cells by LPS also varies with the individual. These suggests that the gingival epitheliums, which has been considered to serve as a mechanical filter or protective barrier, has an immunological function and that individual differences in immunocompetence have some implication in the occurrence, advance and clinical behavior of periodontal diseases. |
