Abstract : 「緒言」欠損補綴においては残存歯および欠損部の配置, 支台歯の負担能力, 欠損部顎堤など様々な要素がその予後を左右する. とくに遊離端欠損症例では, 後方の支台歯が存在しないために義歯の回転軸が増加し, その結果, 支台歯に伝達される為害性のある側方力も増加し, 残存支台歯との調和はとりにくい. これらに対処するために, 従来より補綴前処置としてガイディングプレーン, 近心レストの付与, 歯冠豊隆の修正をして義歯装着時や機能時の側方力の軽減を計る方法がとられてきた. しかしながら, 支台歯が健全歯である場合には, その削除量や支台歯として利用する歯の平行性などのために満足のいく前処置ができない場合が多く, しばしば支台歯に前処置として歯冠修復を施す方法がとられている. これらの要素が遊離端欠損症例の欠損補綴を困難にしているが, 支台歯や欠損部顎堤の動態や圧の分布を解析し, 義歯の設計や支台歯の配置方法に役立てるための基礎的研究が有限要素法, レーザーホログラフィー, 二次元光弾性実験, 接触微小変位センサーなど静的荷重下における解析, 振動を応用した動的解析の両面から行われてきた.

Title : A Study on the Effect of Fixed Splinting Abutment Teeth with Modal Analysis

Abstract : Abstract: An attempt that supports abutment teeth vigorously by fixed splinting using cast restoration has been experimental used in prosthetic design. However, there are few papers that have been studied objectively on the effect of fixed splinting. So the author studied dynamic analysis of abutment teeth by means of vibration on the assumption that are in functional conditions as a method of analysis. The analysis involved the movement of the axis of rotation that will cause by fixed splinting and difference of movement direction of abutment teeth as compare with tooth before preparation. The experiment used lower first and second premolar on right side in model missing lower first and second molar. Three kinds of models were used. The one was used normal teeth before preparation. The other were used preparation teeth that setting single crown and fixed splinting crown made by Pd-Au alloy. Experiment was conducted by giving vibration to the buccal ridge of tooth by taking into account relevant occlusal conditions. The movement of these sample teeth was compared to observe their animation and to measure the degree of displacement through modal analysis. A significant buccolingual rotary motion was observed in all the samples, and the degree of displacement was smaller in the teeth before preparation. A larger degree of displacement was observed in teeth setting the single crown, and still larger displacement as noted in teeth setting fixed splinting crowns. As for mesiodistal movement, displacement of the crown tip was larger than the one of the root tip. On the other hand, the teeth before preparation and teeth setting single crown displayed comparatively large displacement compared to the teeth setting fixed splinting crowns. From these results it has been found that fixed splinting crown restrains mesiodistal movement of teeth effectively, as has been believed before. It also has been found that teeth setting fixed splinting crown can reduce buccolingual movement slightly.
