Abstract : 「抄録」術者や介補者の手指消毒法は, ブラシによる機械的洗浄と石鹸および薬液による化学的消毒法の組合せが, 細菌学的に優れているため, 従来から永く用いられている. しかし, 歯科外来診療においては, 数多くの患者を診療せねばならず, そのため手洗いむらや手洗い時間などについて難点が挙げられる. われわれは, 超音波手洗い装置を用いて手指消毒を行い, 細菌学的検討を加えた結果, つぎのような成績を得た. 1) 0.1% Hyamine (0.1% benzethonium chloride) を1分間超音波洗浄に用いた手指消毒法では, 除菌率は81.4%であり, 2分間応用例では97.1%と除菌率は上昇を示した. 2) 普通石鹸とブラシを用いて1分間洗浄後, 0.1% Hyamine超音波洗浄1分間例では, 除菌率92.3%であった. 3) 10% Hyamineとブラシで1分間手洗い後, 0.1% Hyamine1分間超音波洗浄例は, 99.6%の除菌率であり, この成績は在来本学において用いられているHyamineとブラシを用いる12分間洗浄例の除菌率99.8%に, ほぼ匹敵する成績を示した. 4) 超音波洗浄を応用した逆性石鹸の手指消毒法は, ブラシの刷掃むらの危険がなく, また消毒時間の短縮が図られると思われる.

Title : Effectiveness of Ultrasonic Cleaning in Disinfection of Hands

Abstract : For disinfection of the hands of the dental operater and his assistant, mechanical cleaning with a brush combined with chemical disinfection using a soap or chemicals has been customarily employed for many decades because of its bacteriologically proven efficiency. In the practice of dentistry, however, where a number of outpatients are to be examined and treated every day the combined method of hand disinfection has practical disadvantages of being time-consuming, of effecting irregular cleansing and so forth. This paper presents the results of bacteriologic assessments made of hand disinfection with an ultrasonic hand cleaner. (1) A bacterial elimination rate of 81.4% was obtained by a one-minute ultrasonic cleaning with 0.1% Hyamine solution. When the hands were washed by this procedure for 2 minutes the rate of bacterial elimination was raised to 97.1%. (2) When the one-minute ultrasonic cleaning with 0.1% Hyamine solution was applied after washing with a brush and nonmedicated soap for 1 minute, a bacterial elimination rate of 92.3% was achieved. (3) With the one-minute ultrasonic cleaning with 0.1% Hyamine preceded by brushing with 10% Hyamine solution for 1 minute, the rate of bacterial elimination was 99.6% which is virtually comparable to that (99.8%) normally achieved by the routine procedure at these clinics, i.e. washing with a brush and Hyamine solution for 12 minutes. (4) The date obtained indicate that disinfection of hands by ultrasonic cleaning has no liability to irregular brushing and provides reduction of the time of disinfecting the hands.
