Abstract : [Introduction] From an etiological standpoint, trigeminal neuralgia can be classified into two groups-symptomatic and idiopathic groups. Symptomatic trigeminal neuralgia is caused frequently by dental diseases such as odontogenic inflammation, jaw tumor, cyst and brain tumor. Pain disappears after treatment of causative lesion(s). On the other hand, idiopathic trigeminal neuralgia is thought to be a pathological condition in which no causative lesion(s) cannot be found by various diagnostic methods. Efforts of clinical researchers have been focused on this clinical condition for several years. They have postulated the following theory that a tiny vessels compress the outlet of the trigeminal nerve root from the pons, named root entry zone or exit zone. Currently many patients with idiopathic trigeminal neuralgia are treated and cured with a microvascular decompression (MVD) technique which is derived from this theory. Causative small vessels are moved from the trigeminal nerve root at its entry zone with this technique. Therefore, the terminology as "idiopathic" trigeminal neuralgia should be changed to symptomatic trigeminal neuralgia.
